Aspire Supports provide Individual Supports services under the Community Care program. As with all services, we are dedicated to providing nurturing, therapeutic and person-centered services in and out of your home settings in ways rivaled by few. Every staff member is trauma-focused and is trained in Positive Behavior Supports. Our goal here is to help you achieve whatever it is that you aspire to achieve, including engagement in your own communities and neighborhoods. We are committed to assisting, training and/or supervising you as you learn and perform items such as self-care skill acquisition, social skills training, and growth and proficiency in activities of daily living.
This service is delivered at no cost to you or your family. Under the Medicaid-based Fee-for-Service system Aspire Supports is reimbursed for services rendered. This reimbursement is provided by Medicaid from your DDD individual budget, consistent with prior authorization granted in your individualized service plan (ISP).
Community Based Supports
We provide direct supports and assistance for you in or out of your home in order to help you to reach and/or maintain increased independence, productivity, enhanced family functioning, and inclusion in the community. Our goal remains to help you achieve whatever it is that you aspire to achieve, including increasing your community-based activities and assisting, training and/or supervising you as you learn and perform tasks such as self-care, social skills, and activities of daily living.
As is the case with Individual supports, this service is provided at no cost to you. The reimbursement from Medicaid comes from you DDD individual budget, consistent with the prior-authorization granted in your ISP.
We provide this service outside of your home to assist and support you in educational, enrichment
or recreational activities. Our Trauma-focused staff delivers this direct support service to you in a group of two, three, four, five, but no more than six total participants consistent with the Supports Program Policy and Procedure Manual. Usually provided supplementally to Individual Supports, Community Based Supports or Respite services, Our transportation services are offered to enable you to gain and maintain access to services, activities, and community resources. Generally performed anywhere in New Jersey, New York and Pennsylvania (and within 150 miles one way), our transportation services are delivered on a single passenger basis, free of charge to you.
